EMS Athlete Eligibility

Elkin Middle School invites students/parents/guardians to review all forms listed and print all that apply in this section. If you plan to participate in any sport, the applicable forms listed, along with an updated sports physical and concussion form will need to be completed and submitted to Ashley Davis, Data Manager at Elkin Middle.
 
Middle school 7th and 8th grade students have opportunities to play sports.  These sports include volleyball, soccer, hunter safety, wrestling, basketball, swimming, softball, baseball, tennis, golf, and track.
 
Physicals
All student athletes must have a current physical for every year of participation.  Physicals are good for one full calendar year.  Students may not try out for teams without a current physical on file in the Elkin Middle School office. Forms are available online or in the school office. 
 
Gfeller-Waller Act
Each year all student athletes and a parent or guardian must attend the Gfeller-Waller Act Meeting on state concussion polices.  This is a law we must follow.  Students and parents will then complete and sign the Concussion Form that must be on submitted to the EMS office.
 
Eligibility
To be eligible for participation in a sport, a student cannot be 15 years on or before August 31st.  A student must have been promoted, passed 3 of 4 CORE classes, and been in attendance 85% of the semester immediately proceeding the season of the sport.  A student who does not meet these requirements will face dismissal from the team.  
 
A player must attend school for least half of the day (is counted present) on game day in order to compete.  A player shall not knowingly possess, use, transport, or be under the influence of any narcotic or hallucinogenic drug, amphetamine, barbiturate, marijuana, alcoholic beverage, or intoxicant, or any kind of stimulant or depressant as well as any substance, controlled or uncontrolled.  Neat dress is expected, and profanity will not be tolerated.  Student athletes are subject to random drug-testing.
